Recall Details

Units: About 1,500

Description: This recall involves all Joovy’s Zoom gray metal car seat stroller adapters. The adapters are gray with black plastic clips designed to attach infant car seats to stroller frames. The adapter frame’s dimensions are approximately 17” x 13” x 10”.  Recalled car seat adapter models include 00945 for Graco, 00946 for Chicco and 00947 for Peg Perego frames. “Joovy” and the model numbers can be found on the label at the center of the end bar of the adapter.

Incidents/Injuries: The firm has received nine reports of incidents involving loose adapters on stroller frames. There are no injuries reported.

Remedy: Consumers should stop using these adapters and contact Joovy for a free repair kit to help assure proper attachment to Zoom stroller frames.

Browning Youth Buckmark Junior Hoodie Sweatshirts

Recall Summary

Name of product: Browning Youth Buckmark Junior Hoodie Sweatshirts

Hazard: The hooded sweatshirts have drawstrings around the neck area that pose a strangulation or an entanglement hazard to young children. In February 1996, CPSC issued guidelines about drawstrings in children's upper outerwear. In 1997, those guidelines were incorporated into a voluntary standard. Then, in July 2011, based on the guidelines and voluntary standard, CPSC issued a federal regulation. CPSC’s actions demonstrate a commitment to help prevent children from strangling or getting entangled on neck and waist drawstrings in upper outerwear, such as jackets and sweatshirts.

Recall Details

Units: About 4,300

Description: This recall includes hooded Buckmark junior sweatshirts in sizes S, M, L and XL. The sweatshirts are made of polyester fabric. The camouflaged print pullover sweatshirt comes with black drawstrings and has a yellow embroidered Buckmark logo  located on the front of the garment. There are pockets located near the waist. The sweatshirt has a hood, and rib knit cuffs and waist.

Recall Details

Units: About 305,000 and 36,000 in Canada

Description: The Playtex Hip Hammock is an infant carrier designed to strap the baby against the caregiver’s body at the hip.  It is made of a soft, quilted fabric and intended for babies that are from 15 to 35 pounds.  The child seat is attached with straps that wrap around the carrier’s hips and shoulder. “Playtex Hip Hammock” is printed on a label sewn into the front of the carrier.  All model numbers are being recalled.  Model numbers 05300, 05301, 05302, 05306, 05307 and 05308 are sewn into the inside panel below the instructions for use. They come in basic and deluxe styles. The hip hammock’s fabric is suede or ultra-suede in black or navy colors on the outside, and the inside lining is black, black and white check or burgundy.
Incidents/Injuries: Playtex has received 87 reports of the buckles cracking or breaking, including two reports of injuries, where one infant required emergency room treatment.

Remedy:  Consumers should stop using the carrier and contact Playtex for instructions on how to return the product for a full refund.

Units: About 1,100 sets

Description: This recall involves Edoche’s Cubetensils sets of children’s eating utensils. Each set consists of a spoon and a matching fork with stainless steel heads and plastic handles. The handle has three segments that rotate to create design combinations. The utensils are 5.75-inches long and were sold in seven different models/designs: alphabet, moon and stars, nature, princess, rockets, trucks and veggies. The sets come in a brown cardboard box with a clear viewing window in the front. “Edoche” is printed on the bottom of the utensils and “Stainless Steel China” is embossed on the metal heads.  The phrase “Not for children under 3” appears on the back of the product’s packaging. 

Incidents/Injuries: Edoche has received one report of the utensil’s handle detaching and a baby putting it in his/her mouth. No injuries have been reported.

Remedy: Consumers should immediately take the utensils away from children and contact Edoche for a full refund.

Description: This recall involves girl’s hooded faux leather jackets with a zipper front closure and bearing a Yoki Girls label at the neck. They were sold in girl’s sizes 7 through 16. The jacket comes in black or charcoal, with drawstrings through the neck area. The coats have snap button pockets on each side and a zippered pocket on the top left chest area. Both the black and charcoal colored jackets have a grey hood. Style number JK2503G is on the product care label located on the lower left inside seam.

Incidents/Injuries: None reported.

Remedy: Consumers should immediately remove the drawstrings from the garment to eliminate the hazard or return the garment to Mirage Fashions for a full refund.

Units: About 10,800 in the U.S. and 9,900 in Canada

Description: This recall involves Solowave Tornado brand home playground tube slides with port holes. The slides are green, six or seven feet long and have three or five port hole-style plastic windows in the sides. The slides were sold with Solowave’s Centennial, Centennial II, Lexington, Rocky Mountain Deluxe and Sheridan play systems and were also sold separately.  Solowave Design is stamped on the outside of the slides near the middle of each tube slide section. 
 
Incidents/Injuries: Solowave has received 23 reports of minor injuries to children, including cuts and scrapes, from contact with broken port holes.

Remedy: Consumers should stop using the recalled slide immediately and contact Solowave to receive a free repair kit for the port hole windows.

Sold at: Toys R Us and other stores nationwide, and online at toysrus.com from February 2008 through August 2013 for between $1,300 and $1,800 as part of a play system, or between $275 and $370 for the slide only.

Name of product:  Children’s wall-mounted lamps

Hazard:  A 16-month-old child in a crib died after getting entangled in the lamp’s cord.  In a separate incident, a 15-month-old child in a crib became entangled in the lamp’s cord and nearly strangled. In both incidents, which occurred in Europe, the lamp cord was pulled into the crib by the infants, creating a strangulation hazard.

WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC), in cooperation with IKEA North America, of Conshohocken, Pa., is announcing the recall for repair of children’s wall-mounted lamps due to a strangulation hazard.  A 16-month-old child in a crib died after getting entangled in the lamp’s cord.  In a separate incident, a 15-month-old child in a crib became entangled in the lamp’s cord and nearly strangled. In both incidents, which occurred in Europe, the lamp cord was pulled into the crib by the infants.  There were 2.9 million of the recalled lamps sold in the United States. In addition, 1.1 million were sold in Canada. There was a total of 23 million sold worldwide. 

The recalled IKEA children’s SMILA-series wall-mounted lamps were sold in eight designs, including a blue star, yellow moon, pink flower, white flower, red heart, green bug, blue seashell and an orange seahorse. 
The blue star is the STJÄRNA model with article numbers 501.944.49 or 500.108.79. The yellow moon is the MÅNE model with article numbers 701.944.48 or 700.108.40. The pink flower is the BLOMMA model with article numbers 901.944.47 or 000.979.50. The white flower is the BLOMMA model with article number 300-746-50.  The red heart is the HJÄRTA model with article numbers 202.256.59 or 801.993.13. The green bug is the BAGGE model with article numbers 101.944.46 or 700.728.71.  The blue seashell is the SNÄCKA model with article number 400-982-50. The orange seahorse is the SJÖHÄST model with article number 900-982-43.

The model name is printed on a label on the inside back of the lamp near the light bulb. The article number is printed on the lamp’s packaging. 

The plastic wall-mounted children’s lamps measure about 11 inches high by 11 inches wide. They have a 7 ft. long electrical cord with an in-line switch and take a 25-watt light bulb.

They were sold exclusively at IKEA stores nationwide, in IKEA’s catalog and online at ikea-usa.com from July 1999 through May 2013 for between $10 and $13.

Consumers should immediately stop using the recalled lamp and contact IKEA for a free repair kit.  Do not use the lamp until the repair kit is installed.  The repair kit has self-adhesive fasteners for attaching the lamp’s cord to the wall as well as safety instructions. Name of Product: Water-absorbing polymer toys

Hazard: These soft and colorful products can be mistaken by a child for candy. When swallowed, they can expand inside a child’s body and cause intestinal obstructions, resulting in severe discomfort, vomiting, dehydration and could be life threatening. Similar toys have not shown up on x-rays and needed surgery to be removed from the body.

Description: This recall involves Doodlebutt Jelly BeadZ Jumbo BeadZ and Magic Growing Fruity Fun water-absorbing polymer toys. The toys can absorb from 300 to 500 times their weight in water and can grow up to eight times their original size.

Jumbo BeadZ toys are marble-sized water-absorbing balls. They were sold in a package consisting of three separate 2.5-inch x 2-inch clear, resealable bags inside a 3.5-inch x 4-inch clear, resealable bag. Each smaller bag had eight to 12 water balls of slightly different sizes. The balls came in clear, blue, red, orange, yellow, green and purple colors. The front of the larger bag had a multi-colored label with the words “Jelly BeadZ,” “Easy to follow directions” and had instructions for use.

Magic Growing Fruity Fun toys are water-absorbing polymers in the shapes of apples, bananas, butterflies, cherries, grapes, pineapples, roses and strawberries. They were sold in 3.5-inch x 4-inch, clear, resealable bag with seven assorted shapes in it. They came in blue, green, orange, pink, red and yellow. The label on the front of the bag has the words “For ‘Kidz’ of All Ages,” “Jelly BeadZ,” “Bouncy and Beautiful,” “Colorfast,” “Non Toxic,” “Safe for the Environment,” and other words that describe uses for the product. The back of the package has two smaller labels. One label contains instructions for use and the other has a barcode with “XU00EC1JRN” beneath it.
Incidents/Injuries: None reported. CPSC is aware of one incident with a similar water-absorbing polymer ball product in which an 8-month-old girl ingested the ball and it had to be surgically removed and two cases outside of the U.S. with one death.
Remedy: Consumers should immediately stop using the recalled polymer products and take them away from small children. Consumers should contact Doodlebutt for a full refund.

Description: WASHINGTON, D.C. – The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Angelcare Monitors Inc.®, of Quebec, Canada, is announcing a voluntary recall to provide cord covers for 600,000 Angelcare Movement and Sound Monitors with Sensor Pads. The cord attached to the baby monitor’s sensor pad is placed under the crib mattress, which poses a strangulation risk if the child pulls the cord into the crib and it becomes wrapped around the neck. Angelcare and CPSC have received reports of two infant cord strangulation deaths. In November 2011, a 13-month-old female died in San Diego, California, and, in August 2004, an 8-month-old female died in Salem, Oregon. In both fatalities, the cord from the sensor pads was pulled into the crib by the infant. In addition, there have been two reports of infants who became entangled in cords of Angelcare baby monitor models, which did not result in fatalities. In these incidents, it could not be determined if the “sensor pad cord” or the “monitor cord” was involved in the incident.

The recall involves the Movement and Sound Monitor manufactured by Angelcare. This design of baby monitor includes a unique sensor pad placed inside the crib, under the mattress, to monitor movement of the baby. An electrical cord about 11 feet long is permanently connected from the sensor pad to the nursery monitor unit. The hazard is created by a cord within reach of a baby inside the crib. The cord can be pulled into the crib and can wrap around the child’s neck. The recall involves all versions of Angelcare sensor monitors including model numbers: AC1100, AC201, AC300, AC401 AC601 and 49255 that did not include rigid cord covers, offered in the remedy. The model number is located on the back of the nursery monitor unit. The monitors were manufactured between 1999 and 2013.

Angelcare is providing consumers with a repair kit that includes rigid protective cord covers through which the sensor pad cords can be threaded, a new, permanent electric cord warning label about the strangulation risk, and revised instructions. The recalled baby monitors were sold at Babies R Us/Toys R Us, Burlington Coat Factory, Meijer, Sears, Walmart, Amazon.com, Target.com, Overstock.com and nearly 70 small baby specialty stores, from October 1999 through September 2013 for about $100 to $300.

CPSC Safety Alert Campaign on Monitor Cords

In February 2011, CPSC issued a safety alert warning consumers that industry-wide there had been seven reports of strangulation in baby monitor cords since 2002. Since that alert, the number of death reports has risen to eight of which two involved the Angelcare monitors with sensor cords. CPSC has a safety alert Infants Can Strangle in Baby Monitor Cords (link to latest version 022311) and conducted an information and education campaign with JPMA in which Angelcare has taken an active role to raise awareness on the hazards associated with baby monitor cords. Trail Crest Recalls Children’s Hooded Sweatshirts Due to Strangulation Hazard

WASHINGTON, D.C. - Consumers should stop using this product, which is being recalled voluntarily, unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Recall Summary

Name of Product: Trail Crest boys and girls hooded sweatshirts and jackets

Hazard: The hoodies and jackets have drawstrings through the hood or neck which pose a strangulation hazard to young children. In February 1996, CPSC issued guidelines about drawstrings in children's upper outerwear. In 1997, those guidelines were incorporated into a voluntary standard. Then, in July 2011, based on the guidelines and voluntary standard, CPSC issued a federal regulation. CPSC's actions demonstrate a commitment to help prevent children from strangling or getting entangled on neck and waist drawstrings in upper outerwear, such as jackets and sweatshirts.

Remedy: Replace – Consumers should stop using the recalled blenders immediately and contact Frigidaire for instructions on returning the blenders for a free replacement blender. Remedy: Consumers should immediately take the garments away from children. Consumers can remove the drawstrings to eliminate the hazard or return the garments to Trail Crest for a full refund. Contact Trail Crest for a return address label and instructions for returning the garment.

Description: This recall involves Trail Crest brand boys and girls hooded sweatshirts sold in sizes x-small, small and medium. They have a camouflage pattern on shoulders and arms, with drawstrings in the hood and zippers at the neck. They come in birch and camouflage, pink and camouflage, and purple and camouflage. There is a tag sewn on the outside of the jacket left sleeve with style number 29121. There is an additional Trail Crest tag in the neckline.

Incidents/Injuries: No injuries have been reported.

Description: This recall involves the Journey Girl Travel Trunks used to carry 18-inch-tall toy dolls. The 21-inch tall curved top trunks are purple with a blue pattern and a blue metal handle. The trunks were sold with three clothes hangers and two pull out drawers for storage. Travel trunks included in the recall have UPC # 48970277965070 and model number 5F5F79E. The model number is printed on the bottom of the travel trunk next to the UPC code.

Incidents/Injuries: Toys R Us has received six reports of incidents involving the handle on the trunk, including one report of a consumer who received stitches as a result of a laceration.

Description: This recall involves three styles of The Children’s Place bunny-themed one-piece cotton footed pajamas. One style is pink with dark pink bunnies and a ruffle at the neck. It was sold in size 9-12 months. This style has a sewn-in label at the side seam with both 2158 and one or more of the following numbers: 042521, 042523, 042571, 042572, 042774 or 042816. The other two styles are both blue/green with bunnies on the feet and bunny print fabric. They were sold in sizes 9-12 months, 12-18 months, 18-24 months, 2T and 3T. One style has bunnies with eyeglasses. The other has bunnies and yellow chicks. Both styles have a sewn-in label at the side seam with both 2598 and one or more of the following numbers: 030647, 030779, 038826, 670409, 670602, 670603 or 945210. “Made with love by PLACE” with a heart outline is printed at the neck of the pajamas.

Description: The recalled products are Moose Racing brand training wheels for youth motorcycles and the replacement wheel kit. The training wheels have a black, one-piece, full-length metal axle with a plate that attaches to the underside of the motorcycle frame beneath the engine and two inflatable rubber wheels. The Moose Racing logo appears on the left and right sides of the axle. The replacement wheel kit is a set of two tires mounted on the wheel assembly.
