Deezo Children's Hooded Sweatshirts with Drawstrings Recalled by Zulily Due to Strangulation Hazard

Name of product: Deezo boys and girls zip-up hoodies

Units: About 560

Hazard: The sweatshirts and jackets have drawstrings through the hood which pose a strangulation hazard to young children. In February 1996, CPSC issued guidelines about drawstrings in children's upper outerwear. In 1997, those guidelines were incorporated into a voluntary standard. Then, in July 2011, based on the guidelines and voluntary standard, CPSC issued a federal regulation. CPSC's actions demonstrate a commitment to help prevent children from strangling or getting entangled on neck and waist drawstrings in upper outerwear, such as jackets and sweatshirts.

Remedy: Consumers should immediately take the garments away from children. Consumers can remove the drawstrings to eliminate the hazard or return the garments to Zulily for a full refund. Contact Zulily to obtain a return address label and instructions for returning the garment.

Description: This recall involves a Deezo brand boys', girls', and toddlers' zip-up hoodies made of 65% polyester and 35% cotton. The sweatshirts and jackets have designs that include a heart with arrow, swirl, alien, flower, guitar, motor scooter and circuit board designs and come in the following color combinations: pink with white or black trim; white with pink or green trim; blue with a light blue and white trim and black with blue or green trim.

The following model numbers are included in the recall: 12501, 2502, 12503, 12504, 12505, 12506, 12507, 12508, 12509, 12801, 12802, 12803, 12804, 12806, 12807, 12808, 12809, 12810. Model numbers are located on the care label sewn into the garments side seam? The garment's hangtag has "happyfashion4kids www.deezo.com.au" printed on it.

Hazard: Bean bag chairs without a permanent zipper closure allow young children to unzip, ingest or inhale the small beads inside of the bean bag chair, posing a suffocation and strangulation hazard.

Remedy: Consumers should immediately take the Anywhere Lounger away from young children, inspect the bean bag chair to see if the exterior zipper can be opened and if the zipper on your chair can open contact Powell Company to receive a free Safety Enhancement Repair Kit.

Description: The recalled Anywhere Lounger bean bag chairs are 100% polyester or 100% cotton and measure about 51 inches in height with a 43 inch wide base. Recalled colors include purple (item 199-B004), chocolate (item 199-B005), bayou blue (item 199-B006), pink (item 199-B007), lime green (item 199-B008), denim (item 199-B009), black and white (item 199-B012), striped black and white (item 199-B014), natural (item 199-B016) and camo (item 199-B017). The item number is printed on the product packaging and Powell Company is printed on the label on the bean bag chairs.

Description: This recall involves infant flatware from the Gingham Bunny Flatware Collection, sold three ways; as just the infant feeding spoon, in a fork and spoon set, and in a three-piece set including the infant feeding spoon with a bowl and bib. The flatware is silver-colored, nickel-plated and has a bunny with pink coloring on its ears at the end of the handle.

Incidents/Injuries: Reed and Barton has received one report of the pink coloring on the bunny's ears coming off the flatware. No injuries were reported.

Remedy: Consumers should immediately take the recalled flatware away from infants and contact Reed and Barton for a full refund or free replacement flatware.

Hazard: The vest sold with these sets has a belt at the waist that could become snagged or caught in small spaces or vehicle doors and it poses an entanglement hazard. Description: The recalled overall corduroy dungaree pants were sold in light brown and red in sizes 0-24 months and 2Y through 4Y. There are two snaps on each side of the overalls, side pockets and a center pocket with a sewn-in black and white tab label. There is a label on the back of the waist of the dungarees with item number 72092. The label inside the back strap of the dungarees reads "Baby Boden" and the buttons read "Mini Boden." Pants shipped directly to consumers came in a bag labeled "Mini Boden."

Description: The knit ankle infant socks have a red ankle border with a lime green sock that has four fabric feet and a dark green frog head with black and white eyes and a red tongue stitched to the top of the toe of the socks. The socks were sold with a Cracker Barrel price tag printed with SKU number 417662, S110, size 6-18M, fiber content and care instructions on the back of the tag.

Hazard: Small pegs on the puzzle boards can loosen and separate from the boards, posing a choking hazard to children.

Description: The recalled products are Ryan's Room brand Spin-A-Mals Farm and Spin-A-Mals Safari puzzles intended for children over 12 months of age. Both toys are made of wood. The puzzles consist of a painted, rectangular board with pegs mounted to it and removable gear and animal-shaped pieces. The farm puzzle has 14 puzzle pieces including three animal pieces: a cow, a dog and a sheep. The safari puzzle has 11 puzzle pieces. Children place the pieces onto the pegs and use the knob on one of the pieces or insert an animal figure into other pieces to rotate all of the gears. The puzzle boards have "2012 Small World Toys" on the bottom right.

Incidents/Injuries: The firm has received four reports of pegs separating from puzzle boards. No injuries have been reported.

WASHINGTON, D.C.-- The U.S. Consumer Product Safety Commission (CPSC), in cooperation with six retailers, is announcing the voluntary recall of all Buckyballs and Buckycubes high-powered magnet sets sold by these companies. CPSC continues to warn that these products contain defects in the design, warnings and instructions, which pose a substantial risk of injury and death to children and teenagers.

Imported by Maxfield & Oberton LLC, of New York, N.Y., Buckyballs and Buckycubes consist of sets of numerous, small, high-powered magnets. These sets vary in the number of magnets included and come in a variety of colors. Individual magnets in the set are about 5 millimeters in diameter. Individual magnets in Buckyballs are spherical and individual magnets in Buckycubes are cube-shaped.

About three million sets of Buckyballs and Buckycubes have been sold in U.S. retail stores nationwide and online since 2010 for between $5 and $100.

Consumers should take the high-powered magnet sets and all associated individual magnets away from children and teenagers and contact the retailer from which they purchased the product to obtain instructions for their remedy:

These retailers have agreed to participate because Maxfield & Oberton has refused to participate in the recall of all Buckyballs and Buckycubes.

In July 2012, CPSC staff filed an administrative complaint against Maxfield & Oberton Holdings LLC, of New York, N.Y., after discussions with the company and its representatives failed to result in a voluntary recall plan that CPSC staff considered to be adequate to address the very serious hazard posed by these products. This type of legal action against a company is rare, as CPSC has filed only four administrative complaints in the past 11 years.

CPSC has received 54 reports of children and teens ingesting this product, with 53 of these requiring medical interventions.

Description: This recall involves battery-operated Hot Wheels branded Urban Shredder ride-on toys. The toys were sold in green and black (Model Nos. 8801-05 and 8801-05com) or red and black (Model No. 8801-15) and have Hot Wheels graphics. Recalled models were manufactured on September 15, 2012, October 15, 2012, or December 1, 2012. Model number 8801-15 or 8801-05 or 8801-05com and the date of manufacture, formatted as “YYYY/MM/DD,” are printed on a data label on the underside of the Urban Shredder. The serial number can be found etched or printed on the underside of the Urban Shredder near the data label. Serial numbers included in the recall have:

  • a. letters “CT-EEI” followed by a six-digit number in the range of 000001 through 003075 and 010759 through 011075;
  • b. or the letters “CT-EEJ” followed by a six digit number in the range of 003076 through 010758;
  • c. or the letters “CT-CEJ” followed by a six digit number in the range of 003000 through 005000.
Incidents/Injuries: Dynacraft has received 17 reports of the Urban Shredder toys accelerating. No injuries have been reported.

Remedy: Consumers should immediately stop using the recalled toy, disconnect the battery and return the shredder to the store where purchased for a refund or store credit.

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Gemme Juvenile Inc., of Princeville, Quebec, Canada is announcing a recall to retrofit 300 children's three-drawer dressers. If a young child climbs up open dresser drawers, the dresser can tip over and pose the risk of entrapment. CPSC and the Company have received a report of a two-year-old boy from Barrington, Ill. who reportedly suffocated when he climbed on or up an open lower drawer into the second dresser drawer, causing the dresser to fall and entrap him between the unit and the floor.

When the dresser drawers are pulled all the way out and then the additional weight of a young child is applied, the dresser's center of gravity can be altered and result in instability of the product and consequently tip over. A child can become injured in the fall or suffocate under the weight of the fallen dresser.

This recall involves the Chelsea three-drawer windowed dresser bearing model number 3033. The dressers were sold in five finishes Cappuccino, Cappuccino with a brown top, Ebony, Ebony with a brown top, and Antique or French White. A sticker with the word "Natart" and the firm's logo is affixed to the inside of the top drawer. In addition, most dressers will have the model number, "Natart Juvenile," "Made in Canada" and "Chelsea 3 Drawer Dresser" printed on another label located on the back of the dresser. The recalled dresser measures 35-inches high by 21- inches deep by 39- inches wide and is part of the Chelsea children's bedroom furniture collection. The dresser is composed of engineered wood, solid wood and wood veneers. The top drawer has two clear plastic windows in front.

The dressers were sold at Furniture Kidz and other independent juvenile specialty stores and at Baby.com from January 2005 to December 2010 for between $600 and $900.

The Chelsea three-drawer dresser met applicable standards when produced but was manufactured prior to the existence of the May 2009 voluntary industry standard. That standard requires tip-over restraints that attach to the interior wall, framing or other support be included with all dressers to help prevent tip-over entrapment hazards to young children.

The dressers were manufactured in Canada.

Consumers should immediately stop using and place the dresser out of a child's reach. Free retrofit kits that contain wall anchor straps are being offered to consumers to help prevent the dresser from tipping. WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Bexco Enterprises Inc., dba Million Dollar Baby of Montebello, Calif. is announcing a voluntary recall of 18,000 children's four-drawer dressers. When a young child climbs up on open dresser drawers, the dresser becomes unstable and poses the risk of tip over and entrapment. CPSC and Million Dollar Baby have received two reports of deaths associated with these dressers. An 11-month-old boy from Tulsa, Okla. and a 20-month-old girl from Camarillo, Calif. were reported to have suffocated when their dressers tipped over, entrapping them between the dresser and the floor. The cause of the deaths has not been determined.

This voluntary recall involves "Emily" style four-drawer dressers with model numbers M4712, M4722, M4732 and M4742 and similar "Ryan" dressers with the model M4733. The dressers were sold in five finishes: Cherry, Ebony, Espresso, Honey Oak and White. The model number, "Million Dollar Baby" and "MADE IN TAIWAN" are printed on a label located on the back of the dresser. The recalled dresser measures 33-inches high by 20-inches deep by 40-inches wide and is a part of the DaVinci children's bedroom furniture collection. The dressers are made from pine and wood composite.

The recalled dressers were sold at JCPenney and independent juvenile specialty stores nationwide and online at Amazon.com, BabiesRUs.com, BabyUniverse.com and other online retailers from January 2006 through June 2010 for between $230 and $300.

The Million Dollar Baby dressers met applicable voluntary standards when first produced, but a May 2009 voluntary industry standard, and subsequent revisions published in October 2009 and November 2009, requires that tip-over restraints be sold with the dressers. The restraints attach to a wall, framing or other support to help prevent dresser tip-over entrapment hazards to young children. Million Dollar Baby is offering free retrofit kits with tip-over restraints to consumers who have older dressers. Included in the kit is an adhesive warning label that consumers are to attach to the dresser, which describes how to prevent tip-over injuries.

The dressers were manufactured in Taiwan and the USA.

UPC code 687064045552 or 687064042100 is printed on the product's box.  'Sportspower BouncePro 14' is printed on a plate on the leg of the trampoline frame. The trampolines are surrounded by enclosure netting on the perimeter of the trampoline measuring about 6 feet high. The netting is designed to contain individuals bouncing on the trampoline.

Incidents/Injuries: Sportspower has received an additional nine reports of the enclosure netting breaking since the May 2012 recall. There have been five reports of injuries including broken bones, back and neck injuries and contusions.

Remedy: Consumers should immediately stop using the trampolines and contact Sportspower to receive a replacement enclosure netting.

Fisher-Price Recalls to Inspect Rock 'N Play Infant Sleepers Due to Risk of Exposure to Mold
 
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should immediately inspect this product and stop using it if mold is found. Units currently in retail stores are not included in this recall to inspect.
 
Name of Product: Newborn Rock 'n Play Sleeper(tm)
 
Units: About 800,000 units
 
Importer: Fisher-Price Inc., of East Aurora, N.Y
 
Hazard: Mold can develop between the removable seat cushion and the hard plastic frame of the sleeper when it remains wet/moist or is infrequently cleaned, posing a risk of exposure to mold to infants sleeping in the product. The CPSC advises that mold has been associated with respiratory illnesses and other infections. Although mold is not present at the time of purchase, mold growth can occur after use of the product.
 
Incidents/Injuries: Fisher-Price has received 600 reports of mold on the product. Sixteen consumers have reported that their infants have been treated for respiratory issues, coughs and hives after sleeping in the product.
 
Description: This recall to inspect includes all Fisher-Price Rock N' Play infant recliner seats called sleepers. The sleeper is designed for babies up to 25 pounds and is composed of a soft plastic seat held by a metal rocking frame. The product has a removable, fabric cover that is sold in 14 patterns and color palettes.
 
Sold at: Mass merchandise stores nationwide and online since September 2009 for between $50 and $85. Units currently in retail stores are not affected by this recall to inspect. Only products that show signs of mold after use by consumers are included in this announcement.
 
Manufactured in: China
 
Remedy: Consumers should immediately check for mold under the removable seat cushion. Dark brown, gray or black spots can indicate the presence of mold. If mold is found, consumers should immediately stop using the product. Consumers can contact Fisher-Price for cleaning instructions or further assistance. Recall Nap Nanny Recliners

Four Retailers Agree to Stop Sale and Voluntarily Recall Nap Nanny Recliners Due to Five Infant Deaths
 
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) and four major retailers are announcing a voluntary recall to consumers who own Nap Nanny(r) recliners made by Baby Matters, LLC of Berwyn, Pa.
 
Retailers currently participating include Amazon.com, Buy Buy Baby, Diapers.com and Toys R Us/Babies R Us. At the request of the CPSC, these retailers have agreed to voluntarily participate because the manufacturer is unable or unwilling to participate in the recall.
 
CPSC is warning parents and caregivers that these baby recliners contain defects in the design, warnings and instructions, which pose a substantial risk of injury and death to infants. This recall includes the Nap Nanny Generations One and Two, and the Chill(tm) model infant recliners.
 
In July 2010, CPSC and Baby Matters, LLC issued a joint recall news release that offered a discount coupon to Generation One owners toward the purchase of a newer model Nap Nanny, and improved instructions and warnings to consumers who owned the Generation Two model of Nap Nanny recliners.
 
At the time of the 2010 recall, CPSC was aware of one death that had occurred in a Nap Nanny recliner and 22 reports of infants hanging or falling out over the side of the Nap Nanny, even though most of the infants had been placed in the harness. Subsequently, despite the improvements to the warnings and instructions, additional deaths using the Nap Nanny recliners were reported, including one in a Chill model. Since the 2010 recall, CPSC has received an additional 70 reports of children nearly falling out of the product.
 
The Nap Nanny is a portable infant recliner designed for sleeping, resting and playing. The recliner includes a bucket seat shaped foam base and a fitted fabric cover with a three point harness. Five thousand Nap Nanny Generation One and 50,000 Generation Two models were sold between 2009 and early 2012 and have been discontinued. One hundred thousand Chill Models have been sold since January 2011. The recalled Nap Nanny recliners were sold at toy and children's retail stores nationwide and online, including at www.napnanny.com. All models were priced around $130.

Description: Falls Creek Kids infant and toddler, boys and girls, denim jeans were sold in sizes 12M-5T. "Falls Creek Kids" is printed on a tag sewn on the inside back of the waistband. The jeans are denim with or without a pink star or heart graphic design stitched on the outside front and back of the jeans. The jeans also were sold with cargo style pockets. There is label on the inside of the recalled jeans near the front left pocket that includes the date of production: 04/2012 and production location: Zhangjiagang, China. Falls Creek Kids infant jeans with production code 4/2012 in sizes 0-3 mos, 3-6 mos or 6-9 mos. printed on a label on the inside of the back waistband are not included in the recall.
Sold exclusively at: Meijer and HEB stores nationwide from June 2012 through November 2012 for about $12.

Sassy and Carter's-branded Hug N' Tug Baby Toys Recalled Due to Choking Hazard

Name of Products: Sassy(r)-branded Hug N' Tug Puppy and Monkey and Carter's(r)-branded Hug N' Tug Monkey Toys

Units: About 45,300

Importer: Sassy, Inc., of Kentwood, Mich.

Hazard: The beads inside the clear plastic sphere at the center of the toys can be released and pose a choking hazard to young children.

Incidents/Injuries: The firm has received 12 reports of beads being released from the plastic clear plastic sphere body on Sassy-branded toys. There have been no such reports on the Carter's-branded toys. No injuries have been reported.

Description: The recalled toys for infants can be attached to a stroller or car seat using a ring connector on the toy's head. They have arms and legs that can be easily pulled back and forth for play. The toys are approximately 12 inches tall, made of multi-colored fabrics, contain rattle beads inside a clear plastic sphere body and have the face of either a puppy or monkey. Recalled styles include: Sassy Hug N' Tug Puppy, model #80213; Sassy Hug N' Tug Monkey model #80214 and Carter's-branded Hug N' Tug Monkey, models #61083 that were sold on a blue packaging card and #61540 sold on a grey packaging card. Sassy(r)-branded toys have "Sassy(r)" written on the sewn-in label located on the back of the character's head. The Carter's(r)-branded toys have "Carter's(r)" written in red on the blue plastic ring connected to the toy's head and also the sewn-in label located on the back of the character's head.

Sold at: Mass merchandisers such as Toys R Us and Target, independent specialty stores nationwide and online retailers such as Amazon.com and Carters.com between July 2012 and October 2012 for Sassy-branded models; and from August 2011 to October 2012 for the Carter's-branded models. Carter's-branded models were also sold in Carter's retail stores. The Sassy-branded models sold for approximately $8 and the Carter's-branded models for approximately $14.

Dunecraft Recalls Water Balz, Skulls, Orbs and Flower Toys Due to Serious Ingestion Hazard

Name of Product: Water Balz, Growing Skulls, H2O Orbs "Despicable Me" and Fabulous Flowers toys

Units: About 94,700 in the United States and 600 in Canada

Importer: Dunecraft Inc., of Cleveland, Ohio

Hazard: When the marble-sized toy is ingested, it expands inside the body and causes a blockage in the small intestine, resulting in severe discomfort, vomiting, dehydration and could be life threatening. The toys do not show up on an x-ray and require surgery to be removed from the body.

Incidents/Injuries: The firm received one report of an 8-month-old girl from Humble, Texas who ingested a Water Balz and suffered an intestinal obstruction in August 2011. The Water Balz had to be surgically removed.

Description: This recall involves marble-sized toys that absorb water and grow up to 400 times their original size. They were sold as Water Balz (round-shape), Growing Skulls (skull-shape), H2O Orbs "Despicable Me" (round-shape) and Fabulous Flowers (flower-shape). They were sold in packages of six in green, yellow, red, blue and black colors. "Dunecraft," the name of the toy and the model number are printed on the toy's packaging.

Hazard: The stroller frame can fail to lock in place and collapse while in use, posing a fall hazard to children in the stroller.

Incidents/Injuries: Baby Jogger has received six reports of incidents with the stroller's frame failing to lock. No injuries have been reported.

Description: This recall involves all Baby Jogger City Versa(tm) model strollers manufactured between May 15 and August 20, 2012. The date of manufacture is printed on the stroller's black plastic side hinge below "Baby Jogger" and "City Versa." "City Versa" is also printed on the stroller's fabric in the child's leg area. The strollers have charcoal-colored metal frames with black, silver, red or green fabric stroller seats and canopies.

They have a white metal frame covered by blue or pink mesh fabric and metal arms that extend about 11/2 feet under the mattress. The bed rails measure 17 inches high x 41 inches long. "Dream on Me" is printed on the top rail.

Incidents/Injuries: CPSC and Dream On Me have received five reports involving these bath seats, including a report of a near drowning involving a 12-month-old baby girl. The baby did not require medical treatment.

Hazard: The trampoline's metal legs can move out of position and puncture the jumping area, posing a risk of injury, including deep, penetrating puncture wounds, cuts and bruises to children and adults on the trampoline.

Incidents/Injuries: Sportspower is aware of one incident of the trampoline leg separating from the frame while in use, causing the leg to puncture through the jumping surface. No injuries have been reported.

Description: This recall involves Sportspower Parkside model TR-14FT-COM trampolines. The trampolines are 14 feet in diameter and were sold with an enclosure net. The trampolines have blue or light blue fabric on the safety matting and enclosure pole sleeves. The model number is marked on the packaging and instruction manual. "Parkside" is printed on the enclosure net.

Sportspower Recalls Children's Waterslides Due to Injury Hazard; Sold Exclusively at Menards

Name of Product: Liquid Motion waterslides

Units: About 1,500

Manufacturer: Sportspower Ltd., of Hong Kong

Hazard: The warning labels on the children's waterslide are inadequate for weight limit and fail to tell consumers never to slide head first. This poses a risk of serious injuries to consumers, including neck injuries.

Incidents/Injuries: Sportspower has received one report of a man who received a neck injury after sliding down the waterslide head first.

Description: This recall involves Sportspower Liquid Motion waterslides. Consumers inflate the waterslide with air and spray it with water for sliding. They are used on the ground, not at the pool. The waterslides are blue and yellow and measure about 18 feet long by 9 feet high by 5 feet wide. A "Liquid Motion by Sportspower" logo is printed on the side. Recalled waterslides have item number INF-1375 and UPC 687064031340 located on the packaging.

Sold Exclusively at: Menards Inc. from April 2010 through July 2010 for about $300.

Remedy: Consumers should immediately stop using the recalled waterslides and contact Sportspower to receive new labels to affix to the waterslide. The labels warn consumers not to use the waterslide if they are older than 13 years of age and/or weigh more than 100 lbs. The labels also warn consumers never to slide head first.

Children use their feet to propel the toy.

Suffocation, Entrapment Risks Prompt Recall of PeaPod Travel Tents by KidCo

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC), in cooperation with KidCo Inc., of Libertyville, IL, is announcing the voluntary recall today of about 220,000 PeaPod and PeaPod Plus Travel Beds.

Infants and young children can roll off the edge of the inflatable air mattress, become entrapped between the mattress and the fabric sides of the tent, and suffocate.

CPSC is aware of a death of a 5-month-old boy in December 2011 in New York, New York, who was found with his face pressed against the side wall of the tent. The cause of death was not determined.

In addition, CPSC is aware of six reports and Health Canada is aware of three reports of children who became entrapped or experienced physical distress in the product. Two of the six reports included infants who were found crying underneath of the mattress that had not been inserted into the zippered pocket on the bottom of the tent.

The KidCo PeaPod Travel Beds and PeaPod Plus Travel Beds are small, portable sleep tents marketed for use by infants from birth to 3+ years, depending on the model. The tents have a zippered side for putting in and taking out the child and have an inflatable air mattress that fits into a zippered pocket underneath the floor of the tent. The tents fold into a compact round shape and come with a fabric bag for storage and transport.
 

The following models and corresponding tent fabric colors are included in this recall:

The model number can be found on a small tag on the underside of the product.

The travel tents were made in China and sold at independent juvenile specialty stores nationwide and online at Amazon.com from January 2005 through the present for between $70 and $100.

Consumers should immediately stop using the tents and contact KidCo to get a free repair kit. The kits will vary depending on the model and will be shipped to consumers starting in December 2012. Description: The product is a booster toddler seat, both single and twin, designed to attach to Valco "Tri-Mode" and "Zee" strollers. Only booster seats with batch numbers 3111, 7819, 7822 and 7831 and model number TOD1058, TOD9109 and ZEE0649 are included in the recall. The batch and model numbers are printed on a label attached to the product frame. The seats have a black color fabric seat on white metal base frame.

Dorel Juvenile Group Recalls Eddie Bauer Rocking Wood Bassinets Due to Infant Suffocation Hazard

Name of Product: Eddie Bauer Rocking Wood Bassinets

Units: About 97,000

Manufacturer: Dorel Juvenile Group, of Columbus, Ind.

Hazard: The bottom locking mechanism can fail to lock properly if a spring is not installed, allowing the bassinet to tip to one side and cause infants to roll to the side of the bassinet. This poses a suffocation hazard to infants.

Incidents/Injuries: Dorel has received 17 reports of incidents with the recalled bassinets involving infants primarily younger than three months old. In two of the incidents, infants were reported to have had breathing difficulties after they rolled into the side of their bassinets.

Description: This recall involves Eddie Bauer Rocking Wood Bassinets. The bassinets have a dark brown wooden headboard and footboard and a cotton bassinet that comes in a variety of colors and patterns. "Eddie Bauer" is printed on a metal plate on the bassinet's footboard. These bassinets have a dial at the base of the footboard that locks and unlocks the rocking motion of the bassinet. Recalled bassinets have the following model numbers: 10632, 10639, 10832, 10835, 10839 and BT021. The model number is located under the mattress, on the top surface of the mattress support board or on the wash and care label.

Sold at: Target, Toys R Us and Sears stores nationwide and online at Ebay and other websites from December 2007 through January 2011 for about $150.

Description: The recalled children's upholstered toddler chairs in Club style and Mod style are 24" wide by 18" tall by 17" deep. The Club style chairs come in three fabrics and colors: 1) suede in brown, pink, avocado green, red and turquoise blue; 2) velour in blue, green, pink and red; and 3) print fabric in chocolate dot on pink or blue. The Mod style chairs come in dark pink or zebra printed velour in chocolate on blue or pink; or on printed fabric of chocolate stripes on pink or blue. The Trend Lab name is on a label attached on the bottom of the chair with date codes TL1007C through TL0812C, which are read as TL two-digit month and two-digit year followed by C. These date codes include 10/07 thru 08/12 or October 2007 thru August 2012 production. All chairs were sold with matching ottomans, which are not part of the recall. New chairs with "Remedy 2012" stamped in ink on the bottom side of the chair are not subject to this recall.

Description: This recall involves all Bistro high chairs with model/style number "255" which is printed on a tag attached to the back of the seat. The high chair was sold in blue or pink. The fabric on the seat has a polka-dot design and "Dream On Me" is printed on a label attached to the front of the white tray. The recalled high chair was manufactured in July 2011 and has a date code printed on a label on the back of the chair.

Graco Recalls Classic Wood Highchairs Due to Fall Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Name of Product: Graco Classic Wood Highchairs

Units: About 86,000 in the United States and 3,400 in Canada

Importer: Graco Children's Products Inc., of Atlanta, Ga.

Hazard: The high chair's seat can loosen or detach from the base, posing a fall hazard to the child.

Incidents/Injuries: Graco has received 58 reports of the high chairs' seats loosening or detaching from the base. There have been nine reports of children falling as the seat detached from the base, resulting in reports of bumps, bruises and scratches. Graco has received one report of a concussion in Canada.

Description: This recall involves all Graco brand Classic Wood Highchairs sold in three wood finishes. The high chair has a top seat, bottom leg assembly and removable tray. The high chair is sold with a beige fabric seat cover. Model number 3C00BPN, 3C00BPN TC, 3C00CHY, 3C00CHY TC, 3C00CPO or 3C00CPO TC is printed on a label on the underside of the seat assembly.

Sold at: Babies R Us, Burlington Coat Factory and other retail stores nationwide and at Target.com and Walmart.com and other online retailers between September 2007 and December 2010 for about $130.

Hazard: The water bottle's spout can break off, posing a choking hazard to children.

Incidents/Injuries: H&M has received one report of an incident in England of the water bottle spout breaking off in a child's mouth as the child was drinking from the bottle. No injuries have been reported.

Description: The 16 oz. water bottles are pink plastic with a crackle design or blue plastic. The bottles have flip-top lids in coordinated pink and blue colors. "H&M Sweden" and "www.hm.com" are embossed on the bottom of the bottle. The recalled products were manufactured in June 2012. The water bottle contains the manufacture date in an embossed date clock on the bottom of the bottle. The inner circle on the clock contains the number 12 with an arrow between the two numbers. The arrow points at the number 6 in an outer circle of numbers.

Sold exclusively at: H&M stores with children's departments nationwide from July 2012 through August 2012 for about $5.

Manufactured in: Italy

Remedy: Consumers should immediately stop using the water bottle and return it to H&M for a full refund.

Death of Child Prompts Recall of Window Blinds by Blind Xpress
Two-year-old strangles in cord

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Blind Xpress of Livonia, Mich. is announcing the recall of about 139,000 custom-made vertical and 315,000 horizontal blinds. In 2009, a 2-year-old girl from Commerce Township, Mich. reportedly strangled in the loop of a vertical blind cord that was not attached to the wall or floor.

Blind Xpress custom vertical blinds have an adjustment cord that forms a loop that is not attached to the wall or floor. In some instances, this loop has a weighted device at the bottom. The custom horizontal blinds do not have inner cord stop devices to prevent the accessible inner cords from being pulled out. A child can become entangled in a cord loop and strangle.

This recall involves all Blind Xpress custom-made vertical blinds that do not have a cord-tensioning device that attaches to the wall or floor, as well as all horizontal blinds that do not have inner cord stop devices.

The blinds were sold at various blind specialty stores in Michigan, Ohio and Indiana from January 1995 through December 2011 for between $16 and $380. These blinds were manufactured in the United States.

Summer Infant Recalls to Repair Baby Bathers Due to Fall and Head Injury Hazard

Name of Product: Mother's Touch/Deluxe Baby Bathers

Units: About 2 million in the United States and an additional 65,000 in Canada

Manufacturer: Summer Infant Inc., of Woonsocket, R.I.

Hazard: When the bather is lifted and/or carried with an infant in it, its folding wire frame can suddenly disengage from the side hinge, dropping the baby out of the bather, posing a fall hazard and a risk of serious head injury to infants.

Incidents/Injuries: CPSC and Summer Infant have received seven reports of incidents in the U.S., including five reports of infants suffering head injuries from falls from the bathers. Four children between two weeks and two months old received skull fractures, including one that required intensive care for bleeding on the brain. The fifth child received a bump to the head requiring emergency room treatment.

Description: This recall involves Summer Infant baby bathers with a small, nearly square blue or pink plastic base measuring about 13 ½ inches long by 12 ½ inches wide and with the following model numbers listed below. Model numbers are located either on the side of the baby bather near the warning label or on the front near the wash instructions. Some units have multiple model numbers. Model numbers with an additional letter at the end of the model number are also included in this recall.

The bathers have adjustable side hinges with five rivets each and a white wire frame with a mesh fabric sling seat and two or three positions for the seats. Some also have a head support cushion. The fabric seat comes in various colors including white, blue, green, yellow and orange with fish, turtles, butterflies, frogs, flowers and duck patterns. Bathers manufactured since July 2007 include the warning "Never lift or carry the bather with infant in it."

Sold at: Mass merchandise stores nationwide and on the Web from September 2004 through November 2011 for between $15 and $30.

Baby Seats Recalled for Repair by Bumbo International Due to Fall Hazard

Name of Product: Bumbo Baby Seats

Units: About 4 million in the U.S. Note: In October 2007, 1 million Bumbo seats were voluntarily recalled to provide additional warnings against use on raised surfaces.

Manufacturer: Bumbo International Trust, of South Africa

Hazard: Babies can maneuver out of or fall from the Bumbo seat, posing a risk of serious injuries.

Incidents/Injuries: CPSC and Bumbo International know of at least 50 incidents after the October 2007 voluntary recall in which babies fell from a Bumbo seat while it was being used on a raised surface. Nineteen of those incidents included reports of skull fractures. CPSC and Bumbo International are aware of an additional 34 post-recall reports of infants who fell out or maneuvered out of a Bumbo seat used on the floor or at an unknown elevation, resulting in injury. Two of these incidents involved reports of skull fractures, while others reported bumps, bruises and other minor injuries.

Description: The bottom of the Bumbo seat is round and flat with a diameter of about 15 inches. It is constructed of a single piece of molded foam and comes in various colors. The seat has leg holes and the seat back wraps completely around the child. On the front of the seat in raised lettering is the word "Bumbo" with the image of an elephant on top. The bottom of the seat has the following words: "Manufactured by Bumbo South Africa Material: Polyurethane World Patent No. PCT: ZA/1999/00030." The back of the seat has several warnings and seats manufactured since 2008 have an additional label on the front of the seat warning against use on raised surfaces.

Sold by: Sears, Target, Toys R Us (including Babies R Us), USA Babies, Walmart, and various other toy and children's stores nationwide, and various online sellers, from August 2003 through August 2012 for between $30 and $50.

Manufactured in: South Africa

Remedy: Consumers should immediately stop using the product until they order and install a free repair kit, which includes: a restraint belt with a warning label, installation instructions, safe use instructions and a new warning sticker. The belt should always be used when a child is placed in the seat. Even with the belt, the seat should never be used on any raised surface. Consumers should also immediately stop using Bumbo seat covers that interfere with the installation and use of the belt. A video demonstrating proper installation of the restraint belt and proper use of the Bumbo seat are available at www.BumboUSA.com

Hazard: The metal snap band can wear through the fabric covering resulting in exposing sharp edges and posing a laceration hazard.

Incidents/Injuries: The firm has received eight reports of injuries including lacerations.

Description: This recall includes Animal Snap Bracelets in assorted metallic fabrics with animal figures, including: Seahorse, Frog, Snake, Starfish, Crab, Lizard, Dolphin and Fish. Manufacturing date codes between 07/11 and 01/12 are printed on a sewn-in label found on the inside of the bracelet. The label includes the Toysmith name and address. A round paper tag is attached to the product with the product name, Toysmith information, the words "Item # 2360 Animal Snap Bracelet," SKU number and bar code.

Children's Pajamas Recalled by My Clothes Due to Violation of Federal Flammability Standard

Name of product: Children's Pajamas

Units: About 1,100

Hazard: The pajamas fail to meet the federal flammability standards for children's sleepwear, posing a risk of burn injury to children.

Incidents/Injuries: None

Description: Two types of pajamas are being recalled: a long-sleeved nightgown and a set consisting of a long-sleeved shirt and long pants. The long-sleeved nightgown, Style 14, is cotton knit with multi-colored polka dots. The sleeves and hem are ruffled. The shirt and pants set, Style 15 or Style 314RG, is cotton with a red and white gingham print pattern and white piping on the collar, sleeves and pant legs. The shirt has four buttons. The garments were sold in sizes 12 months through size 14. The style number and the words "PJs by My Clothes" are printed on tags sewn into the center back of the gown, the neck of the shirt and the rear waistband of the pants.

Sold at: Children's boutiques and specialty retailers and online nationwide from August 2011 through December 2011 for between $35 and $45.

Liberty Mountain Recalls VAUDE Kenta Child Carriers Due to Fall Hazard

Name of Product: Kenta and Kenta Plus child carriers

Units: About 300 in the United States and 100 in Canada

Importer: Liberty Mountain, of Salt Lake City, Utah

Manufacturer: VAUDE Sport GmbH & Company KG, of Germany

Hazard: The side strap's seam can unravel and cause the strap to separate, posing a fall hazard to the child in the carrier.

Incidents/Injuries: VAUDE has received two reports of the side strap separating. No injuries have been reported.

Description: This recall involves backpack-style child carriers sold under the Kenta and Kenta Plus model name. "VAUDE" is printed on the back and between the shoulder straps of the carrier. "Kenta" or "Kenta Plus" is printed on the lower back or hip belt of the carrier. The carriers were sold in black, red and brown and were designed for children up to 31 pounds.

Sold at: Outdoor camping and climbing stores nationwide from January 2010 through May 2012 for about $150 for the Kenta and $170 for the Kenta Plus models.

Manufactured in: Germany

Remedy: Consumers should immediately stop using the recalled child carriers and return the product to an authorized VAUDE dealer for a replacement. Consumers can contact Liberty Mountain to locate an authorized VAUDE dealer.

Green Toys Recalls Mini Vehicles Due To Choking Hazard

Name of Product: Green Toys(tm) Mini Vehicles

Units: About 50,000 in the United States and 2,500 in Canada

Manufacturer: Green Toys Inc., of Mill Valley, Calif.

Hazard: The wheels and hubcaps on the toy cars can detach, posing a choking hazard to young children.

Incidents/Injuries: Green Toys has received 10 reports of wheels and hubcaps detaching or loosening from the vehicles. There have been no reports of injuries.

Description: This recall involves Green Toys(tm) Mini Vehicles. All cars are made of plastic and available in yellow, green, red, white and blue. Model names and numbers for the seven recalled vehicles or sets are listed in the table below. For Mini Vehicle (MVHA - 1014) and Fastback (MVFA-1022) sets the model numbers can be found on the bottom of the toys' packaging. For all other cars, a label with the model number can be found on the bottom of the car itself.

Cars with an "I" etched into the underside of the car next to the date stamp are not part of the recall.

Sold by: Mass merchandisers, grocery stores and independent toy and specialty stores nationwide and by online retailers from April 2012 to June 2012. The cars cost about $5 each or $18 for the set.

Peg Perego Recalls Strollers Due to Risk of Entrapment and Strangulation; One Child Death Reported

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Peg Perego USA Inc., of Fort Wayne, Ind., is announcing a voluntary recall of about 223,000 strollers due to a risk of entrapment and strangulation.

A 6-month-old baby boy from Tarzana, Calif. died of strangulation after his head was trapped between the seat and the tray of his Peg Perego stroller in 2004. Another baby, a 7-month-old girl from New York, N.Y., nearly strangled when her head became trapped between the seat and the tray of her stroller in 2006.

Entrapment and strangulation can occur, especially to infants younger than 12 months of age, when a child is not harnessed. An infant can pass through the opening between the stroller tray and seat bottom, but his/her head and neck can become entrapped by the tray. Infants who become entrapped at the neck are at risk of strangulation.

The recall involves two different older versions of the Peg Perego strollers, Venezia and Pliko-P3, manufactured between January 2004 and September 2007, in a variety of colors. They were manufactured prior to the existence of the January 2008 voluntary industry standard which addresses the height of the opening between the stroller's tray and the seat bottom. The voluntary standard requires larger stroller openings that prevent infant entrapment and strangulation hazards.

Only strollers that have a child tray with one cup holder are part of this recall. Strollers with a bumper bar in front of the child or a tray with two cup holders are not included in this recall.

The following Venezia and Pliko-P3 stroller model numbers that begin with the following numbers are included in this recall. The model number is printed on a white label on the back of the Pliko P-3's stroller seat and on the Venezia stroller's footboard.

Consumers should immediately stop using the recalled strollers and contact the firm for a free repair kit. Do not return the stroller to the retailers as they will not be able to provide the repair kit.

Hazard: The front wheel assembly can break, posing a fall hazard to the child in the stroller. In addition, for strollers manufactured in January and February 2012, the nuts that hold the stroller's basket support screws in place can detach. Detached nuts can pose a choking hazard to young children.

Incidents/Injuries: Kolcraft has received six reports of front caster wheels breaking and two reports of the basket's support screws and nuts detaching. No injuries have been reported.

Description: This recall involves all Contours Options LT tandem strollers with model number ZT012. The model number and date of manufacture are printed on a label found on the rear leg of the stroller. The dual-seat strollers have one mesh basket beneath both seats and were sold in two color schemes; black with red canopies and accents, and gray with yellow canopies and accents. "Options LT" is printed on a bar along the side of the stroller.

Sold at: Burlington Coat Factory and juvenile product specialty stores nationwide and online at Amazon.com, Target.com and other online retailers between February 2012 and July 2012 for about $250.

Remedy: Consumers should immediately stop using the product and contact the company to receive free replacement wheels. Consumers with strollers manufactured in January and February 2012 will also receive replacement nuts for the basket support screws.

Incidents/Injuries: Innovage has received 11 reports of short circuiting. This includes three reports of lamps catching fire, which led to property damage. No injuries have been reported.

Description: This recall involves Discovery Kids Animated Marine and Safari Lamps that feature rotating films with marine or safari scenes. The words "Discovery Kids" are printed on the front top-left corner of the product. The recalled products have both an 11-digit batch number that begins with either 584894 or 10128 and a model number of 1628626, 1642433, 1641522, 1641523, 1645729, or 1645853. Batch numbers can be found imprinted in the plastic underneath the lamps and on the bottom of the packaging. Model numbers can be found on stickers placed underneath the lamps and on the bottom of the packaging near the barcode.

Troxel Recalls Flexible Flyer Swing Sets Due to Fall Hazard

Chicco Polly High Chairs Recalled Due to Laceration Hazard

Name of Product: Chicco Polly High Chair

Units: About 455,000 in the United States and 30,690 in Canada

Importer: Artsana USA Inc., of Lancaster, Pa.

Hazard: Children can fall on or against the pegs on the rear legs of the high chair, resulting in a bruising or laceration injury.

Incidents/Injuries: The firm is aware of 21 reports of incidents in which a child fell against the peg and received injuries, including four laceration injuries requiring medical closure (stitches, tape or glue) and one scratched cornea.

Description: This recall involves a range of Chicco Polly high chairs with pegs on the back legs intended for tray storage. The high chairs have a folding metal frame for storage and a reclining seat. The recalled high chairs can be identified by the model number and date code printed on a label on the underside of the seat, close to the footrest. The date code is in the format DDMMYYYY or YYYY-MM-DD. High chairs included in this recall were manufactured prior to October 13, 2010 and have one of the following model numbers on the label:

Old Navy Recalls Toddler Girl Aqua Socks Due to Slip and Fall Hazard

Name of Product: Old Navy Toddler Girl Aqua Socks

Units: About 34,000 in the U.S. and 3,800 in Canada

Retailer: Old Navy, of San Francisco, Calif.

Hazard: This style of Aqua Socks have less traction when worn on wet or smooth surfaces such as hardwood or tile, creating a slip and fall hazard.

Incidents/Injuries: Two customers reported falls with minor injuries that did not require medical attention.

Description: The slip-on shoes have pull-on tabs on both the front and back of the foot opening of the shoe. These Aqua Socks were sold in pink and purple in Toddler sizes 5-11. The recall includes Old Navy Toddler Girl Aqua Socks with style number 896452. A small tag inside the foot opening contains the style number (896452), size information and country of origin (China).

Sold at: Old Navy stores in the U.S. and Canada and at oldnavy.com in the U.S. and at oldnavy.ca in Canada from February 2012 through June 2012. The shoes sold for between $12 and $15.

Manufactured in: China

Remedy: Consumers should immediately stop using these shoes and return them to any Old Navy store for a full refund. Consumers who return the shoes before November 9, 2012 will also receive a coupon for $10 off their next purchase at Old Navy. A white label is sewn into the underside of the orange pads which has "786X Little Jumpers Trampoline" printed above the bar code. This recall involves trampolines with the codes 21011-P0003070, 21011-P0003246, 25511-P0003071, 27811-P0003372, 29811-P0003373 and 34211-P0003375. This code is printed underneath the barcode.

Strollers Recalled by Kolcraft Due to Fingertip Amputation and Laceration Hazards

Name of Product: Contours Options three- and four-wheeled strollers

Units: About 36,000 in the United States and 270 in Canada

Manufacturer: Kolcraft Enterprises Inc., of Chicago Ill.

Hazard: A child or consumer's finger can become caught in the opening formed when locking and unlocking the hinge mechanism which is used to adjust the handlebars on the strollers. This presents an amputation and laceration hazard to children and the adults handling the stroller.

Incidents/Injuries: Kolcraft and CPSC have received five reports of injuries involving the hinge mechanism, including reports of three children whose fingertips were amputated and two adults whose fingers were either smashed or lacerated.

Description: This recall involves Kolcraft Contours Options three- and four-wheeled strollers. Strollers included in the recall have model numbers starting with ZL002, ZL005, ZL008, ZL015 and ZL018. On the ZL002 model, the model number and date of manufacture is printed on a sticker above the left wheel. On the ZL005, ZL008, ZL015 and ZL018 models, the model number and date of manufacture is printed on a label sewn into the edge of back of the stroller seat pad. The strollers were manufactured from January 2006 through November 2009 and sold in various color schemes.

Sold at: Juvenile product specialty stores nationwide and online at Amazon.com, Target.com and ToysRUs.com from January 2006 and June 2012 for between $150 and $160.

Toys R Us Recalls Imaginarium Activity Center Due to Choking Hazard

Incidents/Injuries: Toys "R" Us has received eight reports of the knobs detaching. No injuries have been reported to the firm.

Description: This recall involves the Imaginarium 5-Sided Activity Center. The multi-sided center has two triangle-shaped ends with a wooden multi-colored xylophone on one and a mirror on the other. The three sides are square-shaped and include moveable block letters, rotating gears and sliding shapes. The name "Imaginarium" appears in a blue, oval-shaped logo underneath the mirror on one end. The second 'i' in the logo is dotted with a shooting star. The recalled product has model number 46284; Toys "R" Us item number 295909; and has the barcode number 000799985462841. The model number is printed on the back of the box, directly above the bar code. These numerical markings do not appear on the actual product.

Sold exclusively at: Toys "R" Us stores nationwide and online via www.toysrus.com from August 2009 through September 2010 for about $25.

Safety 1st Toilet and Cabinet Locks Recalled Due to Lock Failure; Children Can Gain Unintended Access to Water and Dangerous Items

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Toilet and Cabinet Locks

Units: About 183,000 toilet locks and 685,000 cabinet locks

Importer: Dorel Juvenile Group (DJG) Inc., of Columbus, Ind.

Hazard: Young children can unexpectedly disengage the toilet locks and gain access to water in the toilet, posing the risk of drowning. The cabinet locks are being recalled because young children can disengage the lock, posing the risk of injury from dangerous or unsafe items stored in the cabinet.

Incidents/Injuries: DJG has received 110 reports of toilet locks that did not adequately secure the lid, including eight reports of children, under the age of two, who were able to disengage or break the lock. In addition, DJG has received 278 reports of cabinet locks that did not adequately secure the cabinet, including 71 reports of children between the ages of eight months and five years old who were able to disengage the cabinet locks. In one of the reported incidents, a 13-month-old boy swallowed small, toxic beads from a craft kit. The child was admitted to the hospital, observed overnight and released the next day.

Description: This recall involves Safety 1st Sure Fit toilet locks with model numbers 48003 and 48103. The toilet lock is attached to the tank behind the lid and is intended to prevent a child's access to the toilet bowl. This recall also involves Safety 1st cabinet slide locks with model numbers 12013 and 12014. The cabinet slide lock is attached to cabinet knobs or handles to prevent access to the contents of the cabinet. Model numbers can be found on the back of the locks.

Sold at: Bed, Bath and Beyond, Burlington Coat Factory, Great Beginnings, Home Depot, Target and Walmart from January 2005 through April 2010 for between $8 and $20 for the toilet locks, and from January 2000 through March 2009 for between $2 and $11 for the cabinet locks. Amazon.com sold both locks through April 2012.

Remedy: Consumers should immediately remove the recalled locks and contact DJG for a free replacement lock of a different model. When removing the recalled locks, consumers are urged to immediately store dangerous items out of reach of children and to prevent unsupervised access to bathrooms.

Hazard: Metal fatigue can cause the handlebar to break away during use, posing a risk of laceration from exposed metal surfaces or other injury from a fall.

Incidents/Injuries: Aqua-Leisure has received four reports of handlebars breaking from the metal connection joint during use. No injuries have been reported.

Description: This recall involves First Fitness Kid's First trampolines with handlebars. The child-size toy trampolines have a red and blue metal handlebar, a blue nylon deck guard and a black jumping deck. "First Fitness" is embossed on the jumping deck in white letters. The trampolines can be identified by model number FF-6902TR and Toys R Us SKN 491463. The model and store numbers can be found on the lower right corner of the back of the packaging. A sewn-in tag on the bottom of the deck lists the factory date code of five numbers followed by "GLTX."

Five Retailers Agree to Stop Sale and Recall Tots in Mind Crib Tents Due to Strangulation and Entrapment Hazard
One Death and Serious Brain Injury Reported

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) and five retailers are announcing a voluntary recall to provide refunds to consumers who own crib tents and play yard tents made by Tots in Mind, Inc.

CPSC is warning parents and caregivers who own these products that infants and toddlers are at risk of serious injury or death due to strangulation and entrapment hazards presented by these products. Tots in Mind recalled the play yard tents in July 2010 and offered a repair kit that is no longer available. The company is no longer in business and has stopped all sales. CPSC staff urges parents and caregivers to stop using these crib tents and play yard tents immediately. Do not attempt to repair these products.

CPSC is aware of 27 tent failures including one fatality and one serious injury that occurred between January 1997 and April 2012 from crib tents and play yard tents made by Tots in Mind, Inc. In 2008, a two year old boy died after becoming entrapped between the bottom rail of a play yard tent and the top rail of a play yard. The fatality was reported in a prior recall with the firm in July 15, 2010.

In 2007, a two year old boy sustained a catastrophic brain injury when the crib tent affixed to his crib tent inverted and the product's broken rod trapped him at the neck. The remaining 25 reports to CPSC involved inverted crib tents-entrapments between the tent and the crib/play yard or failures of the tent fabric and zippers. Three of these 25 reports also resulted in injuries; in one such case, a parent reported finding her child turning blue and entrapped between the product and the top rail of the play yard.

Since Tots in Mind, Inc. is out of business, retailers who sold these products have stepped up to offer refunds or store credit to consumers. The crib tents and play yard tents can present an entrapment and strangulation hazard to infants and toddlers if the dome portion inverts inside the crib or play yard, or if the product becomes partially detached from the crib or play yard. The recalled products were sold at numerous retail stores including Bed Bath & Beyond/Buy Buy Baby, Burlington Coat Factory, Toys R Us/Babies R Us, Walmart and online on websites including Amazon.com, for between $60 and $85.

Consumers should contact the store listed below where the crib tent was purchased to receive either a refund or store credit, depending on the retailer. If consumer is unsure of where the crib tent was purchased, see return policy for individual retailers below on their website:

Description: This recall involves boys' long-sleeved zip-front hooded jackets. The jackets were sold in three colors: army green, aqua green and black and have a fake fur lining. The jackets have one of four designs: the number "2" or the letter "B" embroidered on the left front chest; or the word "ROCK" or "AB Sportwear" embroidered across the chest. The jackets were sold in sizes 4-7 or S, M, and L. Either "SX" or "QH" appears on the label stitched at the neck. The jackets have model numbers: 1006, 1029, 1058 and 1061, however the model number only appears on the original hang tag and not on the product.

Sold exclusively at: dd's Discounts stores nationwide from August 2011 through December 2011 for about $8.

Hazard: The netting surrounding these trampolines can break, allowing children to fall through the netting and be injured.

Incidents/Injuries: Sportspower has received 17 reports of the net breaking, resulting in 11 injuries including broken bones, back and neck injuries, and contusions.

Description: The recall involves the Sportspower BouncePro 14' Trampolines with brown mesh netting. UPC codes 68706404210, and 68706404244 are printed on the trampoline box. "Sportspower BouncePro 14" and "TR-14-63-A" are printed on a plate on the leg of the trampoline frame. The trampolines are surrounded by brown netting measuring about 6 feet high on the perimeter of the trampoline. The netting is designed to contain individuals bouncing on the trampoline.

Sold exclusively at: Walmart stores nationwide from February 2009 through February 2012 for about $275.

Remedy: Consumers should stop using the trampolines immediately and contact Sportspower to receive replacement black netting for the trampoline.

Distributor/Manufacturer/Importer: Kolcraft Enterprises Inc., of Chicago, Ill.

Hazard: The latches that attach the bassinet base onto the metal frame can appear to be locked in place but still remain unlocked. This allows the bassinet to become detached from the metal frame, causing the bassinet to fall and the infant to be injured.

Incidents/Injuries: CPSC and Kolcraft have received seven reports of latches that detached from the bassinet frame. One infant received a bruised cheek when the bassinet detached from the metal frame and landed sideways on the floor with the infant inside.

Description: This recall includes Kolcraft Tender Vibes bassinets with model numbers KB021-ARC, KB022-VER, KB039-CMR1 and Light Vibes bassinet with model number KB043-BNT1. A label with the model/item number of the bassinet is located on one of the legs of the metal frame. The recalled bassinets were manufactured from July 2008 through May 2010. The words "Kolcraft Tender Vibes" or "Kolcraft Light Vibes" is located on the removable music box which is attached to the side of the bassinet.

Sold at: Mass market and independent juvenile specialty stores nationwide and online from July 2008 through May 2012 for between about $50 and $100.

Manufactured in: China

Remedy: Consumers should immediately stop using the recalled bassinets and contact the firm by phone or on-line to receive a repair kit and instructions for securing the latches to the metal frame. In the meantime, parents are urged to find an alternate, safe sleeping environment for the child, such as a crib that meets current safety standards or play yard depending on the child's age.

Manhattan Group Recalls Baby Rattles Due to Choking Hazard

Name of Product: Whoozit(r) Starry Time Rattle

Units: About 3,000 in the United States and 150 in Canada

Importer: Manhattan Group LLC, of Minneapolis, Minn.

Hazard: The clear spheres on each end of the rattle can break, releasing small parts, posing a choking hazard to small children.

Injuries/Incidents: Manhattan Group has received two reports of rattles breaking. No injuries have been reported.

Description: This recall involves Whoozit(r) Starry Time baby rattles. The plastic rattles feature three colored (orange, aqua blue and purple) stars stacked front to back between two clear spheres on a flexible stem. The rattle measures about 5-1/2-inches long. Each clear ball, located at the end of the rattle, measures 1-3/4 inches in diameter. The clear rattles contain small multi-colored beads and a white plastic disc featuring a blue smiling character face inside. The name of the product is printed on the hang tag.

Sold at: Specialty toy and baby stores nationwide, in Canada, and online at www.manhattantoy.com from September 2011 through March 2012 for about $15.

Target Recalls Bunny Sippy Cups Due to Injury Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Name of Product: Target Home Bunny Sippy Cup

Units: Approximately 264,000

Importer: Target Corporation; Minneapolis, Minn.

Hazard: The ear on the bunny sippy cup can poke a child in the eye area while using the cup for drinking, posing an injury hazard.

Incidents/Injuries: Target has received six reports of incidents where the plastic ear poked children during routine use of the product. Cuts and bruises were reported in three of these reports.

Description: The recall involves two styles of Target Home Bunny Sippy Cups. The cups come with handles on both sides in pink and blue, female and male. Each contains a corresponding white bunny head screw-on lid and one bent ear and one straight ear. The cups can be identified by imprints on the bottom: "TARGET 200020683" for pink and "TARGET 200020884" for the blue.

Sold exclusively at: Target stores nationwide from February 2012 through April 5, 2012 for $3.

Manufactured in: China

Remedy: Consumers should immediately stop using the recalled sippy cups and return them to any Target store for a full refund.

Girls' Jackets Recalled by Louise Paris; Waist Drawstrings Pose Entanglement Hazard; Sold Exclusively at Ross Stores

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: "Me Jane" and "B-Hip Kids by Me Jane" girls' jackets with faux fur trim

Units: About 13,000

Importer: Louise Paris Ltd., of New York, N.Y.

Hazard: The jackets have drawstrings at the waist that could become snagged or caught in small spaces or vehicle doors and pose an entanglement hazard to young children. In February 1996, CPSC issued guidelines (pdf) about drawstrings in children's upper outerwear. In 1997, those guidelines were incorporated into a voluntary standard. Then, in July, 2011, based on the guidelines and voluntary standard, CPSC issued a federal regulation. CPSC's actions demonstrate a commitment to help prevent children from strangling or getting entangled on neck and waist drawstrings in upper outerwear, such as jackets and sweatshirts.

Incidents/Injuries: None Reported

Description: This recall involves two girls' polyfill jackets with faux-fur trim around the hoods and drawstrings at the waist. They were sold under the labels "Me Jane Kids" or "B-Hip Kids by Me Jane", and the labels bearing these names are found at the back neck of the garments. The jackets were sold in youth sizes 4- 6X and 7-16. These jackets were also labeled by sizes S, M, L. One style labeled "Me Jane Kids" is fleece with front drawstrings with toggles at the ends and sold in the following colors: green, charcoal grey and black. Other "Me Jane Kids" or "B-Hip Kids by Me Jane" brand labeled bubble jackets have drawstrings on the back, no toggles and were sold in the following colors: purple, black, pink, coffee, and cream.

Sold Exclusively at: Ross Stores, Inc. nationwide from November 2011 through January 2012 for about $20.

Manufactured in: China

Remedy: Consumers should immediately remove the drawstrings from the jackets to eliminate the hazard and contact Louise Paris for instructions on how to receive a full refund.

Youth Jacket Recalled by PUMA; Waist Drawstrings Pose Entanglement Hazard

Name of Product: PUMA USA V-Konstruct Training Jacket with Pockets

Units: About 5,000

Importer: PUMA North America, Inc., of Westford, Mass.

Hazard: The jacket has a drawstring at the waist that has toggles and is not stitched to the back of the jacket. The drawstring could become snagged or caught in small spaces or vehicle doors and it poses an entanglement hazard. In February 1996, CPSC issued guidelines (pdf) about drawstrings in children's upper outerwear. In 1997, those guidelines were incorporated into a voluntary standard. Then in July 2011, based on the guidelines and voluntary standard, CPSC issued a federal regulation. Description: This recall involves the USA V-Konstruct Training Jacket with Pockets model numbers 65110201, 65110202, 65110203, 65110204 and 65110205. The model numbers are printed on the white care label that is stitched into a side seam towards the bottom of the jacket. The model numbers appear to the right of the words "(PVC Free)" and below the words "ART.NO." The recalled jackets have a short collar, full length zipper, two pockets, a mesh panel running along the back of the shoulders and under the sleeves and an elastic cord with toggles at the waist. The PUMA logo is featured on the upper right chest and left sleeve. The jackets were sold in YS (youth small), YM (youth medium) and YL (youth large) and in color combinations red/black, green/black, blue/black, navy/gray and black/gray.

Sold at: Clothing retail stores nationwide and online from January 2007 through January 2012 for between $12 and $55.

Manufactured in: Vietnam, Malaysia and China

Remedy: Consumers should stop using these recalled youth jackets immediately and return the product to the retailer to obtain a full refund.

Additional Fire Incidents Prompts Baja Motorsports to Reannounce Dirt Bike Recall

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

NOTE: Consumers who participated in the March 2011 recall should contact the firm to see if a new repair is required.

Name of Product: Dirt Bikes

Units: About 4,300 (previously recalled in March 2011)

Importer: Baja Inc. d/b/a Baja Motorsports, of Anderson, S.C.

Hazard: The fuel tank can leak, posing a fire and burn hazard to consumers.

Incidents/Injuries: The firm has received 16 reports of fires from leaking fuel, including two reports of minor burns to consumer's legs and one report of a minor burn to a consumer's finger. Ten of the reported incidents, including two of the burn injuries, were received after the March 2011 recall announcement.

Description: This recall involves all model DR50 and DR70 Baja dirt bikes with vehicle identification number (VIN) beginning with "L98," the letter "B" as the 10th character of the VIN and having a yellow dot or line marked on or near the VIN. The model number and VIN are located on the product data plate, which is located on the side of the "goose neck" - where the handle bars meet the body of the bike.

Sold at: Pep Boys and other motor sports stores nationwide from December 2010 through January 2012 for between $450 and $650.

Manufactured in: China

Remedy: Consumers should immediately stop using the recalled dirt bikes and contact Baja Motorsports to schedule a free repair.

Todson Recalls Bicycle Child Carrier Seats Due to Laceration and Fingertip Amputation Hazards

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Topeak Babyseat(tm) II Bicycle Carrier Seats

Units: About 40,000

Importer: Todson Inc., of North Attleboro, Mass.

Hazard: A child can place his or her fingers in the opening at the grab bar's hinge mechanism. When the consumer lifts the grab bar to remove the child from the seat, the child's fingertips can be caught in the hinge mechanism, posing a laceration and fingertip amputation hazard to the child.

Incidents/Injuries: The firm has received two reports of incidents including near amputations that resulted in stitches and a crushed finger.

Description: This recall involves Topeak Babyseat II bicycle carrier seats with model numbers TCS2100, TCS2101 and TCS2102. Model numbers are printed on the product's packaging. The gray, plastic Babyseats were sold in three styles: Babyseat, Babyseat with disc brake compatible rack and Babyseat with non-disc brake compatible rack. The racks are used to mount the seat to the bicycle. "Topeak" is embossed on the back of the seat and is also printed on a black foam protector that covers the grab bar. A red locking lever on the side of the blue grab bar locks the bar into place.

Sold at: J&B Imports, REI, Action and Hawley, independent bicycle dealers, distributors and other retail stores nationwide and online at REI.com from January 2009 through April 2012 for between $140 and $180.

Manufactured in: Taiwan

Remedy: Consumers should immediately stop using the recalled carrier seats and contact Todson to receive a free hinge cover retrofit kit.
