Use of Deadly Force Reporting Instructions and Form
Please use this form to submit information required by ORS 181.789(6)(a), relating to incidents in which a police officer's use of deadly physical force resulted in the death of a person. Do not use this form to submit information about any other death.
DOJ is obligated by ORS 181.789(6)(b) to compile and periodically publish this information; OSP is the "State Deaths in Custody reporting coordinator" and as such, must receive and report the data to the Bureau of Justice Assistance in order to help ensure the state's continued eligibility for certain federal grants.

ORS 181.789(6)(a) requires that you provide at least the information elicited by the first eight questions. These eight questions must be completed in order for Oregon to initiate the record with the Bureau of Justice. Please complete as much of the remaining information as possible in order to comply with federal requirements. OSP may need to contact you periodically (or as a follow-up at year's end) in order to complete the information as it becomes available. You may also provide an updated report later by entering the data on form and marking the box for "updated report".
