Annual Report Requirements

All organizations registered with the Oregon Department of Justice must file an annual report on one of the forms listed below. In order to avoid delinquency penalties, annual reports must be received by the Department of Justice within 4 months and 15 days after the end of the organization's accounting period. For example, organizations with an accounting period which ends December 31 must file an annual report by May 15.

Charitable Activities Forms

Select an annual report form appropriate for the type of charity. If you have any questions regarding the forms, contact the Charitable Activities Section at (971) 673-1880.

If your charity is incorporated or based in Oregon, use Form CT-12.  If your charity is based in another state, use Form CT-12F.  If your organization is a charitable remainder trust, use Form CT-12S.

Charitable remainder or other split-interest trusts
